My child's local school district, in collaboration with the Connecticut Department of Public Health (DPH) is arranging for this testing with Progressive Diagnostics, and DPH is paying for this testing on my behalf. By completing and submitting this form, I confirm that I am the appropriate parent, guardian, or legally authorized individual to provide consent and:

1. I understand that this testing may create protected health information (PHI) of my child. I hereby authorize Progressive Diagnostics, and its subsidiaries and affiliated companies, to disclose such PHI to DPH and my child's school district.
2. I authorize the collection and testing of a weekly PCR molecular COVID-19 test as applicable, on my child during school hours, through this testing program.
3. I understand that the test collection type will be an anterior-nasal swab (a non-invasive swabbing of the lower portion of the nostrils) that takes only a few seconds to collect.
4. Regardless of test results, students MUST adhere to all COVID-19 school safety guidance, including mask-wearing and social distancing, and follow school protocols for isolating and testing in the event the student develops symptoms of COVID-19.
5. I understand that my child must stay home if feeling unwell. I acknowledge that a positive test result is an indication that my child must stay home from school, self-isolate, and continue wearing a mask or face covering as directed in an effort to avoid infecting others. Dates of isolation will be assigned by the district contact tracing team.
6. I understand that the school system is not acting as my child's medical provider, this testing does not replace treatment by my child's medical provider, and I assume complete and full responsibility to take appropriate action in regard to my child's test results. I agree I will seek medical advice, care, and treatment from my child's medical provider if I have questions or concerns, or if their condition worsens. I understand I am financially responsible for any care my child receives from their healthcare provider.
7. I understand that authorizing these tests for my child is optional and that I can refuse to give this consent, in which case, my child will not be tested.
8. I understand that I can change my mind and cancel this consent at any time, but that such cancellation is forward-looking only, and will not affect information that I already consented to be released.

The following items describe what is considered medically necessary testing for the COVID-19 virus. By submitting this form you are agreeing that you meet one or more of these reasons.

1. I am currently experiencing one or more of the following COVID-19 symptoms: Fever between 100.4° F and 102° F, feeling feverish, or feel warm to the touch; shortness of breath, a new or worsening cough, or a sore throat; flu-like symptoms (chills, runny or stuffy nose, headache, body aches, and/or feeling tired); new loss of taste or smell; new nausea, vomiting, or diarrhea.
2. I have been asked to get tested by my healthcare provider, public health department, or a contact investigator.
3. I have been within 6 ft. of a person for at least 15 min. or have been coughed or sneezed on by a person who has been diagnosed with or presumed to have COVID-19.
4. I am routinely in a place where people reside, meet, or gather in close proximity. This includes assisted-living and healthcare settings, office buildings, workplaces, schools, group homes, homeless shelters, prisons, and detention centers.
5. I currently meet one or more of the following high-risk criteria: I am 65 years of age or older; I have been told by my doctor that I am very overweight or obese; I have a chronic condition (e.g., diabetes, high blood pressure, kidney disease or on dialysis, liver disease, lung disease or asthma, etc.); I have a heart condition (e.g., previous heart attacks, heart failure, etc.); I have a neurological condition that affects my ability to cough (e.g., had a stroke); I am pregnant; I regularly use tobacco or nicotine products (e.g., cigarettes, e-cigarettes, vapes, hookah, etc.); I have a condition that weakens my immune system or makes it harder to fight infections (e.g., AIDS, cancer, lupus, rheumatoid arthritis, solid organ or bone marrow transplant, etc.); I am taking medication that weakens my immune system (e.g., steroids, chemotherapy, immunologics, etc.).
